SUMMARY:  Formatting of nested conrefs isn't right
ISSUE:  In a DITA file with nested conrefs, the formatting of the "inner" conrefs may not be properly applied by the EDD. This can happen with the Topic app or the Book app.
SOLUTION:  Just reimport the EDD into the document and the formatting will be properly applied. You don't need to locate the actual source EDD, you can just do the import from the file itself (be sure to remove format rule overrides).

    File > Import > Element Definitions : Import from "Current", Remove Format Rule Overrides

That *should* fix things properly.

Note that if you're seeing this in the authoring view (using the Topic app), you probably shouldn't worry about fixing the problem since it will come back every time you open the file, but fir Book publishing, it's probably a good thing to fix.
